Community nursing care

At Queensland Community Services, we believe quality nursing care should come to you. If you have complex health needs related to your disability, you may be eligible to receive NDIS-funded nursing support in the comfort of your own home. Our experienced nurses are here to help you maintain your independence and manage your health with confidence. It’s also a valuable way to support your loved ones, ensuring they receive the right care for ongoing medical needs – without the need for hospital or clinic visits.

What is Community Nursing Care?

Community Nursing Care is specialised support provided by qualified nurses to help individuals manage complex or ongoing health conditions in their own home or community setting. Under the NDIS, this service is designed to support people with disability-related health needs, ensuring they receive safe, professional care without needing to visit a hospital or clinic.

Accessing Community Nursing Care

If you or someone you care for has complex health needs related to a disability, you may be eligible for Community Nursing Care funded by the NDIS. Here’s how to get started:

Check Your NDIS Plan

Community Nursing Care is usually funded under:

  • Core Supports – Assistance with Daily Life, or

  • Capacity Building – Improved Daily Living

Look for these categories in your NDIS plan. If nursing support isn’t listed yet, you may need to request a plan review or work with your support coordinator to have it included.

Get an Assessment of Your Needs

To justify nursing care in your NDIS plan, you may need documentation from a GP, specialist, or allied health professional outlining:

  • Your disability-related health conditions

  • Why nursing support is needed

  • The frequency and type of care required

Queensland Community Services can help you gather this evidence if you’re unsure where to begin.

Choose a Registered NDIS Provider

Once nursing care is funded in your plan, you can choose a registered provider like Queensland Community Services. As experienced professionals, we:

  • Help you understand your plan

  • Offer a free consultation to assess your support needs

  • Create a tailored care plan that meets your goals

  • Deliver care from qualified, compassionate nurses in your home
